I am an In-Network Provider with:

Health Choice

Blue Cross and Blue Shield

If your insurance carrier is not in my network, you may be eligible for out -of-network benefits. Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part.

I would recommend asking these questions to your insurance provider when you contact them about your eligibility of benefits:

1. Does my health insurance plan include behavioral health benefits?
2. Do I have a deductible? What is my deductible and have I met it yet?
3. Does my plan limit the number of sessions per calendar year?
4. Do I need written approval before I can start receiving services?.

Payment

I accept cash, check and all major credit cards as forms of payment.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session.
